Flinders Medical Centre is seeking a professional, motivated and passionate Social Worker to join our team. As a Social Worker Graduate, you will provide a social work service (including discharge planning) to the patients and families of a designated clinical area at Flinders Medical Centre and will enable you to consolidate what you have learnt during your University Program. Working within one of the major tertiary hospitals in the state, you will provide high quality social work services that maximise patient health outcomes. Using a competent level of professional knowledge and skill, you will provide early assessment of the patient in the context of a multi-disciplinary team approach to care, and develop, negotiate and deliver services that are relevant to the consumers psycho social needs. To be considered, you must hold an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers. With demonstrated experience working with individuals and families, you will also have experience accessing community services and resources. You will have proven ability to use basic computing skills, including email and word processing. In addition, you will possess excellent communication skills with the ability to work collaboratively within a multidisciplinary team and use negotiation skills and collaborate to ensure safe outcomes for patients and their families.
